Intention to Submit 2010-11

All students who wish to submit a TPA must complete an Intention to Submit form. Currently we are asking students to complete the form in both their Spring and Summer seminar courses. Students are to complete four signed copies:
1. TPA Coordinator
2. Site Director
3. Student file
4. Student personal file
It is from this form that I create a spreadsheet to assign assessors TPAs to read.
